As a client of Pretty Moments Boutique, I understand it is possible that at some point during my fitting, services will be provided via ZOOM. 

I understand and agree to the following with respect to the use of Pretty Moments Boutique Virtual services.

1. I understand that these virtual services may involve the communication of my personal information, orally and visually, to Pretty Moments Boutique . 

2. I understand that Pretty Moments Boutique, Virtual services include, but are not limited to, consultation, virtual fittings, and transfer of individual profile details using interactive audio and video. 

3. I also understand that some or all of the laws that protect the confidentiality of my personal information apply to these services, the same as in-person services. As such, I understand that the information disclosed by me during any Pretty Moments Boutique Virtual services are generally confidential. However, there are both mandatory and permissive exceptions to confidentiality.

4. I understand that there are risks and consequences of using zoom services including, but not limited to, the possibility that, despite Pretty Moments Boutique reasonable efforts, the transmission of my individual profile information could be disrupted or distorted by technical failures and/or the transmission of my personal information could be intercepted or accessed by unauthorized persons. I agree that ZOOM is appropriate for my circumstances despite these risks. 

5. It is my responsibility to provide a private location during the Virtual services.

6. I understand that Pretty Moments Boutique Virtual Bra Fitting services may not be as accurate as in-person fittings or services . 

7. I understand that the certified fit specialist is providing Virtual Bra Fitting  and consultation services within their scope of practice.

 

I have read and understand the information provided above. I hereby consent to participate in Pretty Moments Boutique, Virtual services under the terms described above.
