Pretty Moments Boutique Care – Return & Exchange Policy

General Retail Purchases

(Cash, Credit Card, HSA/FSA, Self-Pay, or Self-Submitted Insurance Claims)

  • All sales are final – no returns or exchanges.

  • Exception: We will accept returns only if the item is defective, damaged, or incorrect at the time of delivery, with documented proof.

  • Defective/damaged retail items must be reported within 7 days of purchase.

  • Item must be unused, unworn, clean, and in original packaging with tags attached.

Insurance-Billed Purchases

(Medicare or Any Insurance Payor Billed by Pretty Moments Boutique Care LLC)

  • Returns are accepted within 7 days of delivery for:

    • Items that are defective, substandard, or unsuitable (as defined by Medicare and insurance payors)

    • Unworn items in original condition if the patient is not satisfied

  • Items must be unused unless defective at delivery.

  • Replacement will be provided when possible; otherwise, refunds go to the insurance payor and the patient, as required.

Return Condition Requirements (All Purchases)

  • Unworn and unused

  • Clean and free of stains, odors, or damage

  • In original packaging with tags attached

Patient Responsibility

  • We are happy to assist with insurance questions, but patients are responsible for verifying coverage and confirming our in-network status before purchase

We honor all manufacturer warranties: Silicone breast prosthesis: 2 years: Most athletic forms: 6–12 months

SHIPPING POLICY

Shipping is a flat rate shipping fee of $10.00 within the U.S.

TAXES

All items will be charged the Ohio sales tax of 7.8%
